Join us online on Tuesday, 17 November for a discussion on integrated care in heart failure: lessons learned from the ESCAPE study.

ESCAPE is an EU-funded Horizon 2020 project evaluating an integrated care model for older adults living with heart failure, multimorbidity and mental distress. The project was implemented across five European countries, and evaluates whether a Blended Collaborative Care (BCC) approach, supported by dedicated IT infrastructure, can improve quality of life, care coordination, and healthcare delivery for patients with complex health needs.

This webinar will describe the implementation, methods and results, and explore costings for such treatments from an Irish perspective.
